Disclosure Statement Information

A PSO that enters a Patient Safety Act contract with a health care provider is required to submit a disclosure statement to the Secretary if:

  1. the PSO has other contractual, financial, or reporting relationships with this contracting provider; or
  2. the PSO is not managed, operated, or controlled independently from this contracting provider.

If the PSO does not have any additional relationships or obligations with a provider with which it has a Patient Safety Act contract, a PSO is not required to file a disclosure statement.

Following submission of a disclosure statement, the Secretary is required to determine if the PSO can perform patient safety activities fairly and accurately despite these obligations and relationships. The determination made by the Secretary after reviewing the disclosure statement will be stated on AHRQ's PSO Web site. The disclosure statement, and the Secretary's findings, are public documents and can be obtained by sending an e-mail to PSO@ahrq.hhs.gov.
