A PSO that enters a Patient Safety Act contract with a health care provider is required to submit a disclosure statement to the Secretary if:
If the PSO does not have any additional relationships or obligations with a provider with which it has a Patient Safety Act contract, a PSO is not required to file a disclosure statement. The determination made by the Secretary after reviewing the disclosure statement will be stated on AHRQ's PSO Web site.
